Southwest Virginia Community Health Systems, Inc. (SVCHS) is a Federally Qualified Health Center dedicated to improving access to healthcare for underserved populations across Southwest Virginia. Our Migrant Farm Worker Program provides essential services to agricultural workers and their families through outreach, case management, health education, and care coordination.

We are seeking a dedicated Migrant Health Outreach Worker to serve farm workers across Carroll, Patrick, and Wythe counties. This role works in the field, independently locating and serving farm workers, while building strong relationships with growers, agricultural communities, and clinic teams.

Position Summary

The Migrant Health Outreach Worker acts as a liaison between the community health centers and migrant/seasonal farm workers. This role provides outreach, health assessments, case management, transportation, interpretation/translation, and health education. The Migrant Health Outreach Worker documents all activities and maintains collaborative relationships with growers, community partners, and farm worker families.

Outreach Activities

  • Locate and visit farm worker camps and agricultural worksites within assigned counties.
  • Conduct health assessments and identify healthcare needs.
  • Inform farm workers about available clinic services and community resources.
  • Maintain a Continuity File including grower lists, camp details, and community partner contacts for future staff continuity.

Case Management

  • Interview farm workers to determine needed services.
  • Assist individuals in accessing medical care and community services through referrals, advocacy, and follow-up.
  • Provide interpretation and translation assistance as needed.
  • Provide or arrange transportation when no other resources are available.

Health Education

  • Deliver health education at worksites, camps, or clinic locations.
  • Teach or coordinate prenatal health education sessions when needed.

Other Expectations

  • Maintain strong working relationships with growers, farm workers, and SVCHS staff.
  • Document all activities accurately and promptly.
  • Perform other related duties as assigned to support program and organizational goals.
  • Must be able to adapt to schedule changes or location assignments as directed by leadership.

Qualifications

  • High school diploma or equivalent required; associate degree or relevant training preferred.
  • Bilingual (English/Spanish) required.
  • Experience working with diverse or underserved populations helpful.
  • Ability to work independently in the field, sometimes outside standard business hours.
  • Strong communication, cultural competency, and problem-solving skills.
  • Valid driver’s license and reliable transportation required.
